Insurance and Payment Information for the Contact Dermatitis Institute℠

The Contact Dermatitis Institute accepts the following forms of insurance:

Payments

During your initial visit the doctor will recommend tests appropriate for your diagnosis and care. Payment is required at the time of service. Depending upon your insurance plan, we may bill you or your plan directly. For your convenience, we accept cash, personal checks, and most major credit cards (VISA, MasterCard, American Express, Discovery cards). Because of the expense and time required to prepare the procedures and tests, it is important that we receive 2 business days’ notice if an appointment must be cancelled. Failure to provide notice will result in a $50 “no-show” fee.

Co-Payments

If your insurance company requires a Co-Payment, this payment is due at the time the day of your visit and will be collected upon check in. We accept cash, personal checks, and most major credit cards (VISA, MasterCard, American Express, Discovery cards).
